How VLDL is estimated
VLDL cholesterol is rarely measured directly on a standard lipid panel. Instead, it is commonly estimated with the formula: VLDL = Triglycerides / 5 (in mg/dL), based on the typical ratio of triglyceride content within VLDL particles.
Limits of the estimate
This formula assumes a fasting blood sample and becomes unreliable when triglycerides are above 400 mg/dL, in which case direct VLDL measurement is recommended instead. A normal VLDL level is typically considered to be roughly 5-30 mg/dL, though exact reference ranges vary by lab.
