How LDL is usually calculated
Most standard lipid panels measure total cholesterol, HDL cholesterol, and triglycerides directly, then estimate LDL cholesterol using the Friedewald formula: LDL = Total Cholesterol – HDL – (Triglycerides / 5), all in mg/dL.
A few important limits
This formula assumes a fasting sample and becomes unreliable when triglycerides exceed 400 mg/dL, in which case a direct LDL measurement is needed instead. LDL levels are generally categorized as optimal (under 100), near optimal (100-129), borderline high (130-159), high (160-189), or very high (190+).
